Section C — Declaration

For candidates applying for initiation only

I wish to become a Candidate for initiation and I declare that:

1. My application is entirely voluntary.

2. I understand that my duties as a Freemason would include an overriding duty to obey the laws of any country to which I may be subject.

3. I do not expect, anticipate or seek any preferment or financial benefit as a consequence of my being a member of the Craft.

4. I understand that I may freely declare my membership of the Order on any occasion when I can be seen not thereby to be pursuing any business, professional or personal advantage.

5. Are you, or have you been, a member of any organisation similar to Freemasonry?

You do not need to declare organisations such as Round Table, The Orange Order, Rotary, Buffalo or Lions. If in doubt, please speak with your Sponsor before answering.

Section D — Good Character

Required for all applicants

1. Have you ever been convicted by a Court of any offence?

You do not need to declare fixed penalty fines or convictions that are "spent", but you may if you wish. See: gov.uk rehabilitation periods for guidance, or speak with your Sponsor.

2. Have you ever been disciplined as the outcome of any professional, military, trade or other tribunal?

This refers only to an adverse finding at the outcome of any professional, military or similar regulatory bodies that have the power to prevent you from carrying out the regulated activity (e.g. a Solicitors' Disciplinary Tribunal).

3. Are you awaiting the outcome of proceedings against you before a criminal court or a professional, military, trade or other tribunal?

4. Are you the subject of any criminal, professional, military or trade investigation? (To the best of your knowledge)

If you answered Yes to any of the questions above, you must give full details to your Sponsor and the Lodge Secretary. The Certificate of the Grand Secretary must be obtained before you can be proposed.

If any information supplied becomes untrue or misleading before you become a Mason, you must immediately inform your Sponsor.
